  1. Use the form builder to add sections, questions, conditions, medications, insurance details, and more that suit your services. You can reorder, add, and remove fields. You can add multiple question types, such as short answer, long answer, checkbox, dropdown, date, etc.

  • Decide whether it’s enabled (so it can be used).

  • Enable public access if you want to share via link, embed on your website, or email.

  • Set response expiry (how long a patient’s answers stay valid).

  • Allow translation through Google Translate if needed, so patients see the form in their preferred language.

  • Auto-summarize forms to skim through them efficiently.

  1. If a form is already “in use” (i.e., assigned to patients), and you need to change core fields or structure, duplicate the form first. Work on the duplicate. Then disable the older version.
  1. Once the form looks right, preview it to see what patients will see. Make edits if needed.

  1. If you want the form sent automatically when someone books a service (for example, new patients or specific treatment types), set up Form Automation under Services in Base Setup. Pick which form, set timing (immediately or with a reminder), and save.
