How to Print Blank Forms for Patient to Fill

Here's how you can access and print a blank form to have a Patient complete in person.

From the Template Form Page:

1. Log in as the Admin.
2. Click on the Gear icon (top right header).
3. Under the Template heading, click on Forms.
4. Click on the Three Dots.
5. Click on Print.

From the Dashboard:

1. In the Incomplete Form section, click on the Three Dots on the line of the form you want to print.
2. Click Print.

From the Patient Page:

1. Click on the Patient's name.
2. Click on the Forms tab in the Sidebar.
3. Click on the Three Dots on the line of an Incomplete form.
4. Click on Print.
