Adding a Form from an Appointment

If a Patient needs to update their health information or if you need to send out a specialized form, like a COVID-19 screening form, you can do it directly from an Appointment. You can also send out agreements in your form.

How to Add a Form From an Appointment

  1. From your calendar, click on the Appointment.
  2. Click Add Form.
  3. Select the appropriate Form and Agreements (if applicable).
  4. Click Add Form.
