Current Forms Displayed on the Appointment View

Forms that have been completed will be displayed on the appointment view. When a form expires it will no longer be displayed. If there are no forms displayed (for example, a form that has expired) you can send them an updated form. You can do this directly from the appointment card by clicking Add Form.

  1. Click on the Appointment.
  2. Scroll to the Form section.
  3. If there are no Current forms (because the form may have expired), click Add Form and select the appropriate form.
  4. You can also click View to display any Completed or Incomplete forms.

Important Note: When you click View there are additional functions in the three-dot dropdown menu. In the Completed Form section you can Print or Preview the form. In the Incomplete section, you can Open, Print, Copy, Resend Link, or Delete the form.
