Viewing Current Forms from an Appointment.

Forms that have been completed will be displayed on the appointment view. When a form expires, it will no longer be displayed. If no forms are displayed (for example, an expired form), you can send them an updated one. You can do this directly from the appointment card by clicking on add form.

Viewing Current Forms

  1. Navigate to a Patient's Profile.
  2. Click on the Appointments in the left navigation bar
  3. Scroll to the Form section.
  4. If there are no Current forms (because the form may have expired), click Add Form and select the appropriate form.
  5. You can also click View to display any Completed or Incomplete forms.

Important Note: Additional functions are in the three-dot dropdown menu when you select view. In the Completed Form section, you can Print or Preview the form. In the Incomplete section, you can open, print, copy, resend link, or delete the form.
