Add/Send an Additional Form to a Patient

Practitioners may send additional forms to patients to ensure they update their health history or provide any relevant information for their visits.

  1. Go to a Patient's profile.
  2. Click on the Forms tab in the sidebar.
  3. Click on Add Form button.
  4. Select the Form and the Agreements (if any).
  5. Choose your method of delivery, Email or Complete Now.
