Add/Send an Additional Form to a Patient/Client

You may want to send an additional form to a patient/client, or you may want them to update their health history.

  1. Click on the Patient's/Client's name
  2. Click on the Forms tab in the sidebar
  3. Click on Add Form button
  4. Select the Form and the Agreements (if any)
  5. Choose your method of delivery (Email or Complete Now)
