Enforcing Mandatory Intake Form for New Patients

Some Practitioners or clinics want to make intake forms mandatory before Patients can be accepted into their clinic or allowed to book appointments. This feature requires a Plus or Max plan.

Adding Mandatory Intake Forms

There are two steps to making Intake Forms mandatory. First, a service with the intake form must be established for your Patients to complete. Second, a label requirement must be set for your treatment services to allow patients who have completed their intake forms to view and book appointments.

  1. Log into Admin.
  2. Go to the Patient List icon (header).
  3. On the left sidebar, under Labels, add a Label called 'Intake Form Completed.'
  4. Go to Base Setup (Gear icon).
  5. Go to Services.
  6. Create a service called 'New Patients: Mandatory Intake.'
  7. In the Service Description, you can explain to your potential Patients that this is not an actual booked appointment but time set aside for them to complete their intake form. You can also explain that this is a necessary step for them to be able to see and book the services provided at your clinic.
  8. Set a Duration. You can do something like 15 minutes.
  9. Set up Form Automation using your intake form.
  10. Save Service.
  11. Under Services, Select a Treatment Service you have previously made.
  12. Scroll down to Portal Restriction Labels.
  13. Under the field, Label Required to View or Book, add the label you have created, 'New Patients: Mandatory Intake'.
  14. Save Service.
  15. After someone completes the intake form, you can add the 'Intake Form Completed' Label to their Patient profile to allow them to see and book your services.

Important Note: You can view and resend outstanding intake forms under Forms and Agreements in your dashboard.
