How Do I Complete Outstanding Forms

You might have received a form by email from your clinic, but now you need help locating it. Once you log in to the Web Portal or App, your unfinished forms will be available for you to complete.

1. Log in to your account.
2. Click on Forms in the Sidebar, or Menu in the Footer and then on Forms.
3. Click on any of the Forms that need to be completed.
